Requirements for enrolment include the following:

Duly accomplished Personal Information Sheet (see left)

Birth Certificate

A certified copy of the Permanent School Record and Progress
Report Card, if applicable

Certificate of Good Moral Character if applicable

A copy of immunization records
-
Two 2" by 2" ID photos
Additional Requirements for non-Filipino Citizens (Phases
Learning Center is duly approved to accept foreign
students.)
All applicants will undergo an Assessment Exam to determine
proficiency levels in various subjects.
